The Program of Assertive Community Treatment (PACT) is a community-based, multi-disciplinary team which provides the treatment, rehabilitation and support services needed to enable adults with severe and persistent mental illness to engage in an individual process of recovery.

The team consists of a psychiatrist, nurses, Master's level clinicians, vocational specialists, addiction specialists, a housing specialist, and a case manager. This self-contained team organizes and provides all services in partnership with the participant. Most services are delivered in the individual’s home.

I couldn't have done it without CCBC. If it wasn't for them, I would probably still be out on the streets wandering.

You get a sense of confidence and self-worth. My self-esteem has been steadily increasing, which is pretty cool.

I do know what it's like to be told your behaviour is a problem.
