Current Openings

The Bridge House Program

CCBC is opening a new program serving individuals dealing with both a mental health and substance use disorder (co-occurring disorders) within a Co-Occurring Enhanced Residential Rehabilitation Services (COE/RRS) program.

A mix of clinical and paraprofessional, and recovery specialist staff will be responsible for delivering clinical services, coordinating the individuals treatment plan, providing direct care, coverage and milieu supervision, facilitating a therapeutic milieu through meetings and groups, and addressing care coordination and aftercare needs. Program staff will contain an appropriate mix of LPHA, MA, BA, and Recovery Specialist staff with experience, competency, and/or training in mental health and substance use disorders.


Responsible for providing outreach/case management services to program clients and program service delivery including:

  • Provide range of rehabilitation services; including support groups.
  • Provide range of support services;
  • Provide range of housing development services;
  • Ensure enactment of each client’s Individual Service Plan (ISP) including completion of all required paperwork.
  • Participate in daily organization meetings, ISP review meetings, and supervision, and training;
  • Complete documentation in a timely fashion, including incident reporting;
  • Other duties as defined by agency and/or program leadership
