Description of Position

We are currently seeking a Supportive Housing Case Manager to enhance our current services with those who were unsheltered in our community. This program provides intensive case management to those who have struggled with a variety of disabling conditions including mental health and substance use. This rewarding work allows the opportunity to work one on one with those in our community to help them see their full potential and how to meet that goal while being safely housed. This position calls for frequent transportation of clients and meeting with clients in a variety of settings including public settings, encampments, shelters, etc. As a member of the Housing team you will gain skills and contacts in housing that will benefit you in any social services position.

Responsibilities
  • Coordinate with client, medical, and mainstream providers to ensure appropriate wrap around services in place for client once housed
  • Work with a team to develop programming and supports to help those in Permanent Supportive Housing remain housed
  • Assist with applications for subsidized housing and benefits to increase income
  • Collaborate with local landlords and other providers to establish housing for households in a housing crisis
  • Provide transportation for client to variety of services to ensure needs are met
  • Meet with client in the community to assist with access of services, rapport building, and advocacy
  • Work in the community and in the office, occasional opportunity for hybrid work
  • Work with collateral agencies to locate housing for clients who are currently experiencing homelessness
  • Complete scheduled Needs Assessments and Individualized Action Plans for each client

Qualifications and Educational Requirements

  • Bachelor’s degree in Social Work, Sociology, Psychology, or Human Services related field OR 3-5 years of experience working with homeless populations, and/or homeless programs
  • Knowledge of available referral sources for providing services to clients, including available community support and social service resources
  • Ability to drive throughout the county and at times transport clients required
  • Able to effectively interact with clients, co-workers, and volunteers with diverse ethnic and cultural backgrounds and treat each individual with respect, dignity, and compassion
  • Communicate well both orally and in written form, and follow detailed instructions
  • Comfortable using Microsoft Office 365 tools, and ability to learn variety of computer systems for recording and medical records
Preferred Qualifications
  • Bachelor’s degree in Social Work, Sociology or equivalent education plus experience
  • Multilingual (able to speak and read Spanish, Portuguese, Haitian Creole, Cape Verdean Creole)
