Position Responsibilities

  • Conducts comprehensive treatment planning, completes an individualized action plan and utilizes standard assessment tools with clients upon admission.
  • Provides brief crisis counseling as needed.
  • Provides psycho-education and group counseling daily.
  • Assist in developing and updating a safety plan for each client.
  • Completes required program documentation, forms and mandated reports.
  • Work collaboratively with outside providers such as, outside psychiatrists, therapists, group homes, family members, visiting nursing agencies, etc.
  • Maintain appropriate internal collaboration and consultation with co-workers.
  • Report all safety/maintenance issues to supervisor.
  • Attend all necessary meetings, supervision and trainings deemed appropriate by management level staff.
  • Completes all required documentation in a timely manner consistent with agency guidelines
  • Maintains compliance with all applicable practice standards and guidelines.
  • Maintains client confidentiality and adherence to HIPAA requirements at all times.
  • Performs related duties as required.
