Community Counseling of Bristol County offers community based Partial Hospitalization Program (PHP) services in community based settings for southeastern Massachusetts. These programs provide intensive and comprehensive services for individuals whose mental status has destabilized but who can benefit from skilled outpatient interventions in lieu of inpatient treatment.

The PHP continuum of services offer a supportive and caring environment as a stabilizing base for clients to learn new coping and problem solving skills. Emphasis is placed on understanding one's illness, the factors that trigger an escalation of symptoms, and strategies to manage the illness on an ongoing basis.

Starting at the initial evaluation, individualized treatment and aftercare plans are developed to realize the goal of increasing self-sufficiency and skills to utilize community resources. A psychiatrist who certifies and re-certifies each client's involvement closely supervises the program.

The agency's Elder PHP specifically focuses on the needs and challenges of the geriatric population, paying particular attention to complication factors that frequently arise with the use of multiple medications and co-occurring medical conditions.

Multidisciplinary Team

Each program is operated by an experienced team of mental health professionals, including nurses, social workers, expressive therapists, licensed mental health counselors, and psychiatrists. The program provides psychiatric evaluation, consultation, and medication services on a case-by-case basis. Beginning with each client's admission process, the partial hospitalization team closely monitors the individual's mental status and designs individualized treatment plans and crisis intervention strategies when needed. Each client is assigned a primary treatment coordinator who coaches them on effectively utilizing the program's services, and acts as a liaison to other care providers. The team works closely with primary care physicians and home health nursing providers.
