Description of Position
CCBC is seeking a Family Partner to join the Community Service Agency’s wraparound team for children aged birth through 21 with mental health needs. The FP works with the caregiver of the youth using their own lived experience of raising a child with emotional and/or behavioral needs to teach advocacy skills, connect caregivers to community based and natural supports, provide education on how to navigate through systems, and parenting supports. The FP is part of a multidisciplinary team and should have strengths in cultural humility, time management, knowledge of community resources, and advocacy skills.
Duties and responsibilities of this position include:
- Utilize personal and professional life experiences to provide peer support to youth’s caregivers
- Participate in team meetings to support the caregiver’s advocacy skills with team members
- Engage caregivers in activities that are designed to address goals on the youth’s treatment plan
- Assist caregivers with meeting the needs of the youth by educating, supporting, coaching, modeling, and guiding
- Assist with linking caregivers to peer/parent support and self-help groups
- Teach caregivers how to identify natural and community-based resources
- Provide services in the community
- Complete documentation in line with agency and insurance requirements
Qualifications:
- High school diploma or GED and a minimum of 2 years working with children/families or
- Associates degree in a human service field and 1 year of experience working with children/families or
- Bachelor’s degree in the hum services field and 1 year of experience working with children and families
- Experience as a caregiver of a youth with emotional and/or behavioral health needs
- Must have a dependable car, valid driver’s license, and proof of insurance
- Experience navigating any of the child/family-serving systems and advocating for family members involved with behavioral health systems
