Description of Position

Community Counseling of Bristol County is seeking a full-time Reentry Navigator for the Behavioral Health Supports for Justice-Involved Individuals (BH-JI) program. The BH-JI program provides care management and reentry navigation services for individuals who are involved in the justice system. Individuals will work with our team on transitioning into living a safe and productive lifestyle suited to their needs.

The Reentry Navigator will provide in-reach, transitional and reentry navigation services, along with recovery services, to individuals re-entering into the community from incarceration. Navigators will work with program participants to develop and implement treatment plans that will service and support their transition back into the community, provide supported referrals to address participants’ needs, and to assist program participant to obtain their identified goals.

Requirements

Bachelor’s degree in a related human services experience and/or language ability as well as at least three years of human services related experience of which at least two years of experience has been with the adult mentally ill population including dual diagnosis with addiction. Successful candidates must possess good organizational skills, effective written, verbal and computer skills, as well as knowledge of area resources.
