Description of Position
The CP Director of Care Coordination (DCC) monitors program performance through direct supervision of staff as well as reviews and analyzes data markers to guide strategies that ensure program goals are met. The DCC represents the organization with outside agencies, payers and referral sources, and supports the goals of senior management in all interactions with internal and external customers.
Oversight responsibility for all program administrative functions, including documentation and associated billing, compliance with various payer requirements; monitoring of expenses including mileage, phones, and various office expenses; maintenance of various data collection and data analysis projects and systems to guide program direction; and oversight of remote location supervisors.
Master’s Degree in related field; Master’s in Human Services or Public Health strongly preferred. Minimum 5-10 years of experience in management role, with a number of direct reports, and minimum 10 years of experience in behavioral health field. Experience with remote location management strongly preferred.
Knowledge of Core-Competencies as identified by the employee’s assigned department; must have a valid driver’s license in the state of residence and submit a copy of satisfactory record of driving from the state of residence of registry of motor vehicles, as determined by the department vice president; must complete MA and ADP Criminal Offense Record Information (CORI) process; and must have computer literacy as related to the requirements of the position.
