COMMUNITY CONNECTIONS DAY REHAB PROGRAM


Community Connections is an adult day rehabilitation program serving adults struggling with mental illness and/or addictions. Through a variety of rehabilitative and educational activities, the program promotes symptom management, recovery, functional and social skill acquisition, and prevocational skill development.

The program offers a comprehensive continuum of psychosocial rehabilitation services including: small group activities, 1:1 life skill instruction, prevocational work units, life skills workshops, and social/recreational activities. The program's seven-stage model of psychiatric rehabilitation focuses on empowering clients to identify and achieve goals in their preferred living, learning, working and social environments.

  • Rehabilitation readiness groups
  • Prevocational training opportunities via Flexible Activity Units and Work Enclaves
  • Independent Living Activities training and workshops
  • Basic Adult Education and computer instruction
  • Wellness and Recovery classes and groups
  • Dual Recovery programming
  • Drop-in Center/Social Club

This process includes:
  • Developing rehabilitation readiness
  • Establishing a rehab goal
  • Functional assessment
  • Developing a rehab plan
  • Work and learning experiences
  • Measuring progress
  • Transitioning to other network supports

The program's approach recognizes and accommodates those with psychiatric illnesses who are at various stages of their recovery and therefore require different rehabilitation approaches and alternatives.

Dual Recovery Anonymous (DRA)

Community Connections sponsors nationally sanctioned DRA meetings led by member leaders with consultation from the program's recovery specialist. These free-standing groups provide a forum to any person who experiences the dual issues of mental illness and addiction. Meetings occur on Wednesday and Friday evenings at 6:00PM and are open to the public.

Social Club

Community Connections provides social and recreational activities to members with both on-site and community-based events. Community trips, dances, board games, movies and videos are regularly scheduled activities.

Day Rehab Services: Monday - Friday, 8:30AM - 4:30PM
Social Club: Wednesday & Friday, 4:30PM - 8:00PM; Saturday 9:00AM - 5:00PM
