HIV CASE MANAGEMENT AND HEALTH-RELATED SERVICES


Community Counseling's HIV Case Management and Health-Related Services programs include: Case Management Services to over 130 individuals living with HIV and Housing Advocacy to the Greater Taunton and Attleboro areas.

Living with HIV can be challenging, not only for the person infected, but also for the ones he/she loves. Typical difficulties include depression, marital/partnership and family problems, loneliness, rejection, fear and anxiety. We offer a comprehensive program of counseling and support to those who have been diagnosed as HIV positive, whether symptomatic or asymptomatic, and to their family members and/or significant others.

  • Therapy/support groups for HIV positive persons

  • Food packaging services
  • Psychiatric evaluation and medication services
  • Coordination with Morton Hospital infectious disease physicians

While the person is faced with medical problems and the issues associated with negotiating the medical system, along with societal acceptance and advocacy issues, emotional difficulties are often intensified. Our program is designed to provide therapeutic counseling as well as case management services to assist beyond the initial diagnosis and referral. We work in conjunction with a consortium of community health, human and social service providers who are committed to the prevention of, education about and treatment of HIV and AIDS.

As a long-time affiliate of Morton Health Foundation which until recently operated Morton Hospital and Medical Center, Morton Home Health Care, and Morton Physicians Associates. CCBC continues to utilize Morton Physicians Associates, whose infectious disease specialists work closely with CCBC's HIV Case Management Services Program and bring expertise and substantial experience in the treatment of those with HIV/AIDS. This affiliation has resulted in the integration of behavioral health into the network of primary and specialty medical care at the local level with the capacity to respond to the community's health care needs with sensitivity, flexibility and efficiency.
