Licensed Mental Health Counselor: Community Support Program

Seeking part-time (20 hours per week) Licensed Mental Health Counselor (LMHC) for agency’s Community Support Program (CSP). CSP is a short-term program which is offered by certain Medicaid providers to provide intensive case management services to clients who are considered to be "at risk" within the community. This program offers a single point of contact for coordination and follow-up on a client's individual care. The Community Support Providers are mobile, flexible, and support clients in their community.

The CSP offers support for clients who have had multiple hospitalizations or poor treatment experiences. Some clients are dually diagnosed with mental health and substance abuse issues, or are pregnant and struggling with addiction, or have suffered a catastrophic event (e.g. homelessness, death of a loved one) that puts them at risk of hospitalization, or have significant medical co-morbidity. Staff works side by side with clients to help them develop the necessary resources they need to live successfully in the community. This may include a housing search, connection to clinical services, application to social security, or transportation to medical appointments. This position requires some direct service as well as supervision of some staff.
